Providing consent

There are 3 ways of making your wishes known.

1. Have your decision recorded in the Registre des consentements au don d’organes et de tissus of the Régie de l’assurance maladie du Québec (RAMQ).

This form is automatically sent by mail, along with the notice of renewal of the health-insurance card, for all people who are not already entered in the register.
